If using Chrome, after completing the form in the browser, you
MUST CLICK THE PRINT ICON in the upper right corner – In the
“Destination” tab choose “Save as PDF” – then Save.
If completing the form in any web browser, you MUST save (top right corner) to your desktop when completed. Then upload the saved document on this page down below.
If these steps are not done, your documents will be received blank!

STEP 2: FILL OUT FORM & SAVE TO YOUR COMPUTER OR PRINT IT OUT TO TAKE TO THE SCHOOL
All students are required to have copies of birth certificates, shot records, and Social Security cards on file at the school. High school drivers are also required to have copies of their drivers licenses on file at the school. If your child does not have these documents on file, you will need to take them up to the school prior to the first day or your child will not be able to attend classes.

If enrolling your child in Virtual Learning, you will need to fill out the following application in addition to the traditional forms above:

Please click on your child’s grade level to help them select the classes they wish to enroll in for the 2020-2021 school year. If Mrs. Boeckman has already emailed your child’s link directly to them and they have already filled this out, there is no need to do it again.
