If using Chrome, after completing the form in the browser, you
MUST CLICK THE PRINT ICON in the upper right corner – In the
“Destination” tab choose “Save as PDF” – then Save.
If completing the form in any web browser, you MUST save it to your desktop (save is usually top right corner) when completed. Then upload the saved document on this page down below.
If these steps are not done, your documents will be received blank!

All students are required to have copies of birth certificates, shot records, and Social Security cards on file at the school. If your child does not, you will need to take those required documents up to the school prior to the first day or your child will not be able to attend classes.
